I, the undersigned, grant permission to my child to attend the Greater Grace Community Church Camp Life Jr. event on Saturday, July 27th from 10 a.m. until 2 p.m. I understand that this is a day full of physical activity and that, when I grant my child permission to participate in these activities, Greater Grace Community Church, its staff, and its volunteers cannot assume responsibility for the safety and welfare of the children beyond making a reasonable provision for their supervision.
I also understand that children younger than 3 years and 10 months and those older than 7 years and 2 months of age will not be considered Officially Registered even if their names have been submitted. This is due to our supervision restraints and the designs of the activities involved.
Your signature below represents your (1) consent for your child to participate in the field day; (2) acknowledgement that there are certain risks of injury, allergic reaction, property damage loss, or other harm that may arise from your child's participation in the activities; (3) agreement to accept general liability for the participation of your child in the activities; (4) agreement to waive, release, indemnify, and hold harmless Greater Grace Community Church, its staff, and its volunteers from any and all claims and liability, including but not limited to costs, expenses, and attorney's fees, by reason of injury, allergic reaction, loss, or other harm, arising out of, in connection with, or in any manner related to your student's participation in the school activity as described below.
IN LOCO PARENTIS: By signing below, you also give Greater Grace Community Church staff and volunteers permission and authority to administer general first aid to your child, should they require immediate and minor care.
